There are a number of Vehicle Missions in Grand Theft Auto: San Andreas, with this page detailing the Paramedic one.

The Paramedic is one of seven Vehicle Missions in the game, and involves you finding patients to bring to the hospital. In order to begin this side mission, you will need to find an Ambulance, with some being parked at the hospitals. Also, if you happen to kill enough people, an Ambulance might show up, but that also puts you at risk at being arrested/killed by police. Once you have an Ambulance, simply hit the indicated button to begin the Paramedic side mission. The full Paramedic side mission is split into 12 levels and in order to complete it, you will need to do all 12 levels in a single sitting. The side mission starts with you picking up a single patient, then it will add another patient for each level.

Of course, you will have a timer and each patient you pick up will add to the timer, with the amount added depending on their distance from the hospital. That means at Level 12, you will be picking up a total of 12 patients before being finished with that level. The problem is that the Ambulance can only carry three passengers, one in the front passenger seat and two in the back. With that knowledge, you will see you need a total of four trips to the hospital on the final level. Also, you are required to pick up the maximum passengers per level, so you can’t pick up two, bring them to the hospital, then pick up another two. You will have to pick up three, then the final one for that level, as an example.

Where To Do Paramedic

You have access to the Paramedic side mission as soon as you begin the game, but it’s not ideal to do it in Los Santos, as it is big and the rival gangs don’t exactly make things easy for you. You will need to keep the same Ambulance throughout the entire thing, which means you have to drive very carefully and also make sure you don’t accidentally tip the vehicle over. In fact, none of the three major cities are prime targets for the Paramedic mission. The best place to do this side mission is in the countryside little towns. Once you get to Angel Pine in the Whetstone area after completing The Green Sabre, that should be when you do it.

The two main reasons for this location are that it’s small, and it’s entirely flat. Angel Pine is ridiculously small as a town, meaning that you won’t have to travel far to get patients or to the hospital. The only problem with it is that the timer will be low, but you shouldn’t need to worry about that at all. The other good thing is that Angel Pine is flat, so you don’t need to worry about hills and possibly tipping over the Ambulance. There’s also no one out there that will be attacking you, so the only way you’ll be damaging the Ambulance is if you do it yourself.

Paramedic Rewards

As you complete each level of Paramedic, you will be rewarded with money. For just completing Level 12 alone, you will get $7200. For completng all levels, you will also receive an additional $5000. The main reward for the Paramedic side mission, though, is your health will be increased to the max. It can slowly increase on its own, but Paramedic is the fastest and easier way to increase it.
